Illness Form Use this form when your child is sick and cannot attend school.
Medication Administration Authorization Form Use this form when your child needs to have medication administered at school or at a school event.
Parent Driver Information Form Use this form when you are driving children other than your own in your car for a school event.
School Health Form You do not need to use the Cornerstone Form linked here. You may, but the dated report from your pediatrician showing your immunization record will suffice.
NH Division of Public Health Services Religious Exemption Form Please use this form if you choose not to have your child immunized. Please note that this form must be notarized. Our administrative assistant is a notary, so you can have your form notarized right at school.
Records Release Form Please use this form if you are coming to Cornerstone and you need your previous school’s records released to us.
